What forms would I need to file with the court to obtain custody of my god-daughter?

I live in Pinellas County, Florida. I am trying to obtain non-contested custody of my god daughter. Her mother is unable to care for her. The agreement will be permanent, however, she does not want to relinquish parental rights so adoption is not an option. I am unable to find the doents I would need and "heaven forbid" the courthouse actually be of any istance. No one there seems to have ANY idea. Seems they only have Temporary Petition for Relative Caregiver or a Petition for Guardianship where there are 20 steps to summons, appeal, yada yada. This is uncontested, I just need legal rights given to me to care for her.
